SACRAMENTO — Sutter Medical Center, Sacramento’s Community Navigator Program was honored as a finalist at Jackson Healthcare’s National Hospital Charitable Service Awards.

Teresa Olivas, a community navigator, works with the homeless to provide access to health care and other services.

Jackson Healthcare instituted the Hospital Charitable Services Awards in 2010 to identify high-impact community health programs, share their stories and offer these innovative programs as models for other communities to replicate.

SMCS’ Community Navigator is offered in partnership withSacramento Steps Forward and is designed to connect with our homeless population to ensure they have access to primary health care through a medical home, housing, food, medical insurance, community resources, transportation and substance abuse treatment.

This program has been extremely successful in extending the reach of SMCS beyond the walls of the hospital and into the greater community, to ensure that linkages are drawn between the often complex medical and social needs of Sacramento’s chronically homeless population.

“We have found that the best way to better our community is to develop unique partnerships that deliver care at the right time and right place,” said Holly Harper, Sutter Health Community Benefit manager. “The Community Navigator helps us provide important preventive care and resources to our community’s most vulnerable population.”

In 2015, the Community Navigator made contact with more than 120 homeless individuals, with 40 percent being successfully linked to housing and 45 percent to mental health treatment.

Additionally, starting later this month, SMCS will connect a WellSpace Health “street nurse” with the Community Navigator to provide on the spot medical care for primary and preventive needs.

Sutter Auburn Faith Hospital Recognized by Healthgrades for Superior Clinical Outcomes

Posted by on Oct 20, 2015 | 0 comments

Sutter Auburn Faith Hospital (SAFH) today announced that it has been recognized by Healthgrades for clinical excellence in Treatment of Stroke and Pulmonary Care.

Healthgrades is the leading online resource helping consumers make informed decisions in order to find the right doctor, the right hospital, and the right care. This achievement was released today with other new findings in the Healthgrades 2016 Report to the Nation, which evaluates hospital performance at nearly 4,500 hospitals nationwide for 33 of the most common inpatient procedures and conditions.

HealthgradesThe new report highlights the disparity in hospital performance for specific conditions and procedures both nationally and regionally. This variation in care has a significant impact on health outcomes. For example, from 2012 through 2014, if all hospitals as a group, performed similarly to hospitals receiving five stars as a group, on average 222,392 lives could potentially have been saved and 166,086 complications could potentially have been avoided.*

A five-star rating indicates that SAFH’s clinical outcomes are statistically significantly better than expected when treating the condition or performing the procedure being evaluated.

In addition, SAFH was also announced as a five-star recipient for the treatment of the following conditions:

  • Heart attacks
  • Congestive heart failure
  • Hip fracture
  • Sepsis

“The Healthgrades recognitions highlight the incredible team of health care professionals we have at Sutter Auburn Faith Hospital,” said Mitch Hanna, SAFH CEO. “We are lucky to serve a great community and we continue to work to enhance our services to meet the community’s needs. Over the last several years we’ve focused on high quality care and have been recognized as a Joint Commission Accredited Stoke Center.”

About the Awards
For its analysis, Healthgrades evaluated approximately 40 million Medicare inpatient records for nearly 4,500 short-term acute care hospitals nationwide, and assessed hospital performance relative to each of 33 common conditions and procedures. Healthgrades recognizes a hospital’s quality achievements for cohort-specific performance, specialty area performance, and overall clinical quality. Individual procedure or condition cohorts are designated as 5 star (statistically significantly better than expected), 3 star (not statistically different from expected), and 1 star (statistically significantly worse than expected) categories. The complete Healthgrades 2016 Report to the Nation with detailed cohort-specific outcomes data, hospital-specific quality achievements, and detailed study methodology, can be found at www.healthgrades.com/quality.
